I believe there has been a fanmade 3D model of the scout's mom that respect very well the style of TF2. Maybe try to find where you can get ahold of it and make a render and do a paintover with it instead of drawing it manually.

Ah, perhaps you could have some cigars inside it then?
It would be a logical solution as to what would fit the empty space inside the cigar case.
Also are you going to do anything about the taunt or are you going to let Valve handle it, potencially letting it have an unfitting taunt like the paintrain, or no taunt at all like the Skullcutter?

I could have sworn I read somewhere that it was disguise as a Radio; and as it doesn't look that far from it, I didn't question the idea of it being disguised as a radio when seeing it in game :O

If you're alright with that, then there's not much to worry about.
Most of the community just thinks it looks lazy having the normal taunt as it looks very strange with anything but the bottle, but then again many other thinks the strange taunt is awesome !!
Any suggestions on textures? I really don't know how to paint a cigar texture, after looking through ton of images I still don't know how to implement them with a TF2 feel! Are the current textures good?
I'd suggest to try exporting/compiling your models so you can at least see how they look in the model viewer and have a better look with Texture using TF2's shading.
I'm not a fan of the button labels, I think it would look better if the buttons were blank (maybe different colours). The button functions seem meaningless, really, and don't seem to have any relevance to the actual function of the device.

Just use the same sort of brushwork on the cigars you would on a wooden stock in TF2 - broad-ish strokes in a painterly manner. I'm really not that concerned about your cigars, they look okay as is, just think they could be more TF2ish.

I'm not sure if this is what you had in mind, but it almost could be an old cigar box that the Spy added a little tv-thing to.
One little thing you could add? A little golden clasp on the flap. It wouldn't even need an animation- with as often as he opens and closes the thing, he probably wouldn't bother with the clasp- but it would look very cool.

I like the gold plating better, I do think it does become a little overbearing (maybe make it a little more unsaturated) but when I look from the first to the second and back to the first, I can see there is something missing.
