Well. I'm not a huge fan of the multi-barrel design. However, it would make life a lot easier for me if I made the proportions of this thing fit the joints of the old grenade launcher since it has bones and animation for the reload action. A multi-barrel design would let me do that while giving it a different look.
Right now I'm rather partial to 3 and 5. I'd like to have the barrels be a solid piece instead of having a gap between them if I do something like these.
I would like 7 the best if the sight was moved towards the front more. Most of the other silhouettes look a bit too busy. It also looks like it could make good use of the current animations while still having its own unique look.
I did a quick paintover of a real grenade launcher and incorporated some design elements from some of the concepts above. The proportions need a bit of work if I want to use stock animations, but I like where this is going.
I'd say you should make the stock curve downwards more. With a weapon that is that long, wide, and tall that stock just would make it hold oddly. Generally weapon stocks curve down below the center of balance of the weapon to help offset the weight.

I just copied the stock wholesale from a reference image of a grenade launcher. It does look weird until you realize that's how they actually are.
I made a few changes to this thing. I've also included the original image I painted over for comparison.
Right now I'm trying to balance design with function. Well, not "how would this work in real life" sort of function, but more just adjustments so that it will fit the poses and animations required by the game and the bone setup.
Looks like you are off to another great start Swizzle. The nade launcher looks very cool so far, my only gripe is that the top rail is pretty boring at the moment. Adding something similar to the sight thing on thumbnail 1 might be a nice way to break up that long flat top surface.
Consider for a moment WHO this weapon belongs to, the demo man is a Drunk, and his current weapons look beat up, and worn down. I suggests you make that double barrel look like it was hand made. like he duck taped that barrel together. Hum?
Looks hot Swizzle, I'm super glad youre doing a second pack, gives me something else to keep my eye on . It would be very helpful to see a picture of your draw up next to the regular pipe launcher, because as it is, your design is good, but looks a bit underwhelming, but that just might be me, I'm sure the model will look fantastic

I was able to decompile the grenade launcher, delete unnecessary bones and then have the remaining ones animate a mockup model perfectly in the model viewer. I was actually kind of surprised that it worked, but it's a very good sign.

Yeah, I was a bit nervous about the stock until I took a close look at the animations for the grenade launcher. As long as I don't get too wacky with the proportions, I should be fine.

Actually, the only ones that look really beat up are the Targe and the Eyelander. The rest are actually fairly clean. Hell, the grenade launcher is practically pristine!

Like others have said the bottom version makes the most sense, but I don't like the vertical cut into the bottom barrel or the large empty area to the right of the trigger. Try extending the wooden grip further to the left and removing the vertical line cutting into the barrel.
I think the thickness of the bottom one works the best. When the barrel connects to the bottom part (forgive me, i know nothing about the mechanics of guns) it gives the launcher a real sense of weight, which I think is nice.
I did a quick blockout to see how this thing looks in 3D. I think I need to beef up the barrels a bit. There are also a couple of very minor clipping issues with the stock, but those are easily fixed.
I also imported the blockout as a grenade launcher replacement. All the animations work perfectly. Woo!

So if it's an over/under instead of a revolver loading weapon how will the original animations match?

The concept you have right now looks perfect, and if the stock doesn't clip just that by itself would be great to have in game.

Have a look at the barrel/stock area. Which one sucks least? I'm leaning towards the middle option.
The top one looks too thin and the bottom one looks too thick.
Also, it looks a bit like a giant Force a' Nature !!
It only bears a resemblance though, it would still look awesome in game I'm sure.
