Its 18+ because of "sexual content" and "nudity". This is only the sexual content. The nudity is from the demons and druid in the game, who are essentially naked chicks, with a strand of hair covering their nips.

Yeah, iD Software still own the content (which is largely why projects like OpenArena use all their own content). I suspect all three applications are dubious.
Nope, there no FaceFx anymore. You can use 3DS Max or even Blender for exporting content (animation, static, etc.). The FBX file format is still used for importing content.

This is an art content/art direction issue and has nothing to do with PBR or any technical aspect of rendering. Whether you make boring or interesting content is entirely up to you.
