What you describe is exactly how to do it, so it's probably a Maya problem not a you problem> I've had some tools mess up randomly Maya before, including extrude. Just have to delete the user preference files and reset Maya back to default.
If you can, save your keymapping and what not before hand so you don't nuke all your settings, shelves and what not.
It is also possible that you've extruded it after extruding once and not moving the vertices (therefore having 0 width faces on the edges). as Ghogiel said it should work just like that
