What I did was, in ZBrush add a layer open the mouth, used that to retopo and bake, later you can close the mouth on the lowpoly. Would be interested in how others handle that

A mix. Simply using a tiling texture for each mat type would work, but it's low effort and kind of meh looking a lot of the time. We changed up how this works for Toolbag 5. You can set the UV channel for the material in the Texture panel. If you need different maps to use different UV channels, you can do that with layered materials. There's a ton of additional flexibility in this case too, you can use blend modes to multiplity, overlay, etc…

Are layers involved? Maybe try scaling it at the highest subdivision level to see if that makes a difference. Possibly even going up to the highest, using Makepolymesh3d, and then scale that (reconstruct the lower levels later).
