Did you make sure your high poly was in the same location? In blendee hit ctrl+G location, then set that to 000 for both objects.

Adjust the pack margin to something like 0.03 (depending how big your map size is) AFAIK, the functionality isn't implemented in 2.5 as of yet

You need to mess with distance and bias. Probably your bias is too low. If it's the default .001 try setting it .01 and rebake, or .1 and rebake.

Yeah you are going to want to use rand(0,3) and if the rounding doesn't work you could always implement that yourself with some simple if statements.
