For TF2 characters or props, I'd first find or make some concept art.
Block it out in a 3d package or sculpting program.
Create a base mesh in the 3D package, sculpt if there's anything that needs to be sculpted (for TF2, there's very little that would be sculpted, some people like to do heads/faces in zbrush and repto them, but don't get too detailed, your not baking out normal maps).
Create the final mesh with all the detail, UV map, and then work on the textures.
3DS is currently the most popular software for games, but there's no reason not to try out the other ones and see what you like. Maya tends to be a bit more popular for animation and in other cg industries. Blender is also a free option, but isn't used very often in games.
You'll also need a 2D program like photoshop or the free option, gimp.
