Finalizing Your Entries

Ok, so it's generally known that to submit your Polycount entry, you have to do so through the Contribute Site... Now, I believe there is a form of some sort that must be filled out in order to be an official entry. Can someone point me in the correct direction to find said form? Also, is there anything else we should know before submitting our items?

As far as I know, it's just filing in this template in the "Anything else we should know" section on the Upload Page.

IMPORTANT: Copy & paste this template for the “Anything else we should know?“ section of your submission. There is important information your notes MUST contain to be eligible for this contest. Using this template will ensure those details are given properly.

there's no form there's just the "anything we should know" tab

people generally make a little picture to show/explain their items prior to testing
