I decided to start working on custom items for the spy, since he wasn't getting as much community love, and I had a few interesting ideas for possible items. Right now I just have concepts but once my finals are over I'm going to start working on concept sketches and get those the way I want them before I start going 3-D.
Right now I have:
The Devilish Derringer(name is a WIP but I kinda like it)-
I thought since the Derringer is traditionally a stealthy weapon, it would fit the spy perfectly. Now when the spy botches back stab attempts, usually he has to pull the revolver and run, often with less than favorable results. With the Devilish Derringer's special bullets, the spy would have more of a chance to get away successfully. While it would only have 2 shots at most, and be much weaker than the regular revolver, the Devilish Derringer would slow the targets movement on hit, allowing more breathing room for the spy to cloak and make a successful escape. The DD could also be used to slow and confuse targets before backstabbing them before they even know whats going on. It would sacrifice the offensive power of the revolver to allow for more defensive options.
I think a Derringer fits the spy given its historical use in assassinations, and the fact that it's easy to conceal, making it something a spy wouldn't find as a hindrance. I was thinking something fancier than most normal Derringer designs to make a clear distinction from the scout and engineer's pistol. I also might add a skull and crossbones warning somewhere on the gun, just to give it more of its own personality.
The Fat Lady:
The Fat Lady is a fancy cigar case that would replace the disguise kit. Even though the Spy is a globetrotting rogue, he is cultured and sophisticated and seems like the type of Man who knows good opera, and good cigars. The Fat Lady would be a cigar case, featuring Brünhilde, the famed Valkyrie and titular fat lady. The case would have a regal look to it, and seem very valuable. It would have only one cigar, but with quality like this one is all you need.
While the cigar case would look normal on the outside, the inside would fold out revealing an expensive state of the art radar system, made with plans stolen from the engineers notebook. Since radar was relatively new in the time period of Team Fortress 2, the device would have to look old, with a retro style antenna, and not very detailed mapping of your area (it would show the player, and their position relating to others, but no geographical information).
This radar system would let the spy know where everyone on both teams were located in his immediate area, and when it would be safer to move or stay still. This would replace the spy's ability to disguise at will, instead automatically disguising him as the last person he backstabbed. If a spy botches an attempted backstab, they would lose their disguise and have to work for that next identity.
I know Valve has the ultimate say in what the items will do but I think it helps to work around themes and ideas, and that they'll be stronger for it. I think the spy's knife is fine the way it is, and there are already two cloaks, so I tried to tackle other issues the spy has as a class with different weapons.
My last idea was for a hat based on the Hat/Mask combination of the pulp villian Fantomas. I think the spy might have been a fan of Fantomas seeing as he was a popular fictional european theif, and the spy might have felt a kinship with him.

didn't think of that, and its a cool idea. The only problem with it is that valve doesn't ever add new animations, so I would be stuck with the spy just pulling it out normally.
Yeah that's the kind of thing I'm definitely looking at, the pictures I put up are just to get out the general idea of shapes and sizes.
Right now the problem I'm seeing with the Fantomas hat would be that the top hat would look similar to the Scotsman's stovepipe. If I keep going with that I would need to find a way to make it distinct.
