I'm having a problem in max when pelt mapping that I've never seen before. It only appears to happen on boxes or meshes derived from boxes which I find very odd. When the pelt map function to unwrap a box in max9 the uv window looks like this:

and stranger still, it also happens in max2009 but appears differently:

any ideas what could be causing this?

My first guess, would be something that chuggnuts tools overwrote.
Second guess, would be some other plug-in or script.
Third, possibly a video card driver incompatibility, does it happen in software mode or OpenGL?
The only possible work around I can think of, make another type of primitive, snap it to your box mesh (so the pivot doesn't change), attach the box to the new mesh and then unwrap.
