this looks more like zbrush shuffled the UVs. apply the lettered checker inside ubrush to check if it is in fact that.
if it is, go into Tool - UV Map - Adjust - Cycle UVs, hit it a few times until it looks okay.
now you might think its done, but it is not. press it 2 more times until it looks okay another time. if you have triangles in your lowpoly, the first seeminlgy okay state, will not be okay
Thanks, though the problems started before I ever imported/exported from Zbrush. I tried your above suggestion, but it did not fix the problem, unfortunately.
Really looks like all your UVs triangles are reversed tho, maybe due to some matrix funkyness in your process. I guess if you quickly unwrap the model again, it fixes the issue ?
Try reseting the normals and using smoothing groups instead of the custom normals. You'll know the normals are unlocked if they are dark blue when you reset them

Does the model have overlapping UVs? If so, try translating the overlapped UVs one unit in the U direction.
No overlapping UV's. It uses a second UDIM for the mirrored geometry.
