Height map for 3d print?

Hey there I have a client who wants 3d printed tokens for his tabletop game he is making and he wants cracks on them, so I thought could I use Substance Painter to do that with just a height map like this?

I usually use Blender for my modelling but the final outcome in Blender is very jagged (and the beveled edges have become a mess) and looks low quality even with a 16 bit, 8k, png height map on a 3.5mil face model that is uv unwrapped correctly.

See here...

Anyone done this before and have any tips?

Thanks in advance

The only way I can see something like this working is if you take that height map of the cracks into zbrush and use it as an alpha to sculpt the detail in. It's kind of a pain in the ass but it's the only way that I've been able to do something like this. There is a feature in Simplify3d that will allow you to use a vector image to create a 3d object, but it's very finicky and will only support .svg or .ai files and they can't have a gradient.
