I have had this problem before and I was able to fix it by going back and smoothing through my subdivision levels, but that seems like kind of a jinky fix.
I retop my mesh in Zbrush, bring it to maya, make sure my mesh is clean, make sure all my verts are welded and normals are facing the correct direction. I reimport to Zbrush, subdivide and project with the options shown in the image. Every time I try this I get problem areas as indicated in the image. This has been a major roadblock in my work flow and is VERY annoying and frustrating.
Does my "Mean" and "PA Blur" need to be adjusted in someway? I adjusted the Dist and I have adjusted the Projection Shell before, and still come out with the same problem.
I don't know if I am doing something wrong in the retop phase or what? I have watched many tutorials and no one seems to be having this problem.
Like I said this has been a very VERY frustrating problem that I feel can probably be fixed easily with something that I am over looking or unaware of. Please Help

It happens often during the reprojection. Like... almost always ; )
But the polish/smooth/resculpt technique has always worked fore me.

ould you go into just a bit of detail on the use of deformation/polish?
One the right you see the "Deformation" tab, right? Well, click it and you see some functions.
Polish basically relaxes or smoothes the party that are stretched topologically (is that even a word?!). For example, let´s take a sphere that has ONE vert moved far away from its original place. Polish will have the strongest effect on taht one vertex, trying to put it back in its original place. Im am not sure if my explanation is correct, but it is how I see it + if you press Ctrl and move your mouse over the polish function, it says something like that ^^
You will instantly see what polish does, if you take a sphere/box/anything and strongly move/stretch a part of it and use polish.
