mirror the animation by parenting to a dummy and position constrain new bones to the mirrored ones. or you can do it properly using complicated maths -

you could try using an extra joint for the elbow itself ... and use joints parented to the main joints to tidy deformation. this maya , or max? -

What do you mean when you say 'connected'? The mesh needs to actually be skinned to the bone and not just parented or constrained or some such, just to be clear.
