if you outsource to an individual for a specific job (eg a hero character) then they'll charge you a day rate - it'll be around the same sort of rate you'd expect to pay a skilled tradesman (plumber/electrician). It's not very common but when games do use pregnant characters they tend to be like that... plot gimmicks. The same goes for sci-fi/fantasy movies I don't actually think it would be a very memorable piece.
