Extrude a poly plane to make your roadways. After than, extrude edges outwards to make the rest of the terrain. Make cuts where you want more details or stuff that isnt flat, like holes. Yeah not sure about this one Phil. I would say redo the whole thing if you are gonna rework it. Right now it feels too soft to me. It doesn't seem like a face but rather something made of playdoh. When are you venturing back to San Diego? I'm stuck in Maine for another week and a half. It's like hillbilly hell here.
4-5K budget should mean you won't need so many polygons in the teeth themselves. Spend them elsewhere, unless you want to extrude some molars. To be honest, you'll probably not see so far into the mouth. Make sure you have at least one entire edgeloop INSIDE the mouth to get soft shapes when animating.

I find it awfully hard to make that many vertices form a good curve without much deviation, mostly because it's not that easy to check whether they actually form the curve and which way they'd have to be moved to correct that.
