If you're working on a grid based system then put the pivots somewhere on the grid. Keep it consistent between similar parts so you can swap them out easily. No, all movement anims should start with the same foot forward. Unity has a system where you can tag left foot / right foot contacts, but I'm not sure about Unreal.
