it's never failed me unless theres' been something wacky about either the model or the rig make sure you set the reference frame in skin properly, resetting xforms would be sensible too.
turn off always defeorm in sdvanced settings of the skin modifier, position everything and turn it back on. it helps if you do all this at a frame where it's in the bindpose
