The way I've seen it done (and done myself) is to use a "mouth bag", with the teeth and tongue sitting inside there as you'd expect. One way to do this would be to alter your basemesh to have the mouth bag and teeth/tongue, and work on them as separate polygroups or subtools. If you don't have a basemesh you can do this…
