I think o2 is on to something. Did you parent the hand mesh to the bone as well as skinning it? -

A simple trick is to parent the joint to the object, then zero out its translation, and unparent it back to the world. -

dont have the plate be part of the same bone as the rest of the gun, it should be its own bone/parent, only the bolt should rotate?
