Best guess is double Faces. Great way to see them is turn on select face from center in preferences.

On the low-poly object, go into Face sub-object, select all, Hide Selected Faces. Then try again.

It doesn't do any occlusion. It will select all faces that are facing the front view, regardless of whether they are behind others.

Yes, Maya averages different inbetween faces that are quads and faces that are triangles. I believe it averages to the adjacent edges?

id say change your bias, distance and check your face normals such as if some are facing inwards etc.
