That seemed to be broken for me as well in 2014. My fix was to just extrude it a very small amount in world space, then I was able to use local to extend it out further...Crappy for sure. I also ran into issues with my Gizmo becoming "distorted". Local extrude is working as intended in Max 2015, though!
Unfortunately, I cannot scale it up and then back down because that requires entering 0.01 into the scaling dialog, which results in me scaling it by 0.0100000015. What version of 3ds Max are you using? It's changed quite a bit recently. If you are using 3ds Max 2011 or earlier search for Reactor tutorials on YouTube. If you are using 3ds Max 2012 and up then search for MassFX tutorials. Both systems can do what you want very easily.
Its better in 2015 However. I have been using the layer pallete to organise my scenes since they forced the new (slow) scene select dialog on us. You can leave it open all the time and on 2012 its like a million times faster than the normal scene select window
