The problem you're running into is happening because you are trying to chamfer poles, that's where more than 4 edges connect together.
3DS Max likes to chamfer loops, with only 4 edges or less. So you have to take steps.
Yea, I have seen your stuff before. I got a question; Does it fit into the modeling workflow well? can you undo the whole operation, or does undo only undo parts of it at a time? are there fast live-previews for the sliders? Does it break any compatibility with anything?
I have the Quad Chamfer script and while I like it a lot, one irritation is that it seems to want a single Edit Poly on the top of the stack or it refuses to work. That gets in the way sometimes. You can't have, say, your "Cube" modifier as the base of your object with an Edit Poly above that and apply a Quad Chamfer - it'll just refuse to do it. You have to collapse to an Edit Poly object first.
That said, everything else you mentioned is handled. You can undo, it fits into the workflow like any other modifier, and it does have a good live preview.

I use Modo to model and it works fine. It has that "Quad Chamfer" type of chamfer though.
