I don't want to sound bad, but I can't help but see pigtails with red ribbons coming from the skull. I think maybe experiment with changing the angle of the skull and or adding another skull bellow ish. Looking great all the same keep it up!
I see what's the problem, but first it's not ribbon, but flesh (I'm concious that's not obvious on my drawing, but in the final mesh, it would be great) and second, maybe, if delete some flesh to see a little more the bones , it could be solve the problem...

I like it. To get that flesh feel, maybe have it stretching across more parts of the axe. To me, it feels like it's just kind of laying on the blade. More tension?
good idea but you've got some major issues here, first being proportions. The actual blade of the axe is tiny in comparison to the skull and makes the weapon as a whole seem tiny. You are using WAAAAY too many triangles on the skull, let the normal map pull it's weight and use the triangles elsewhere. The handle of the axe is also far too straight, it feels very CG, make it a little wonkie or have it taper slightly, try to avoid straight lines. Lastly your textures and sculpts are quite noisy, try to simplify in places, specifically the head of the axe (which feels very low poly) and the section between the skulls jaws.
