Once you have an alpha you could use a stencil or paint it in with projection master as well inside zbrush. Drag Rectangle + alpha would also work, depending on what your trying to accomplish.
I would take that image into max, trace out a few shapes with a spine and build some simple base meshes, nothing fancy, just make sure they hold their shape and are roughly even quads and have edges in the important places.
Then take those into zbrush/mudbox/3dcoat/sculptris and knock in some of the smaller details.
It might help to unwrap it and use the image as a guide as to where to sculpt the details. I'm not sure you'll get enough sub-divisions or a alpha brush that works like you want, but who knows it might work. IF not the details aren't that complex that they can't be sculpted.

Separating the thing into smaller pieces sounds like some really good advice, so I'm not overwhelmed by its complexity and can concentrate on smaller parts first.
