The thing next to the submit button aka quiz answer should be 3.

The bottom is as following:

1st line - Nickname
2nd line - Name
3rd line - Korean Phone Number (018-XXXX-XXXX) X = Random Number
4th line - Email
5th line - Captcha

Check Box is for getting korean AMD emails.

After you are done, the page goes white and closes after it gives you a prompt.. then this place usually does batch files to send the codes to the emails, so expect this to be in your inbox in a couple of days or so.

AMD sends these emails in batches so you might get them in a couple of days.
