2011 at work, but we plan to switch to 2012 by the end of the summer, we're on subscription so its not a big deal. We are waiting for the current project to wind down and for 3rd party developers to update their plug-ins and scripts and for Autodesk to release SP1.
At home I'm using 2012 for most things modeling/unwrapping, but I have 2011 and installed in case I drag work (animation) home with me.
My studio, Tencent Boston, is on 2009 and we're making an MMO so I doubt we'll be updating anytime soon.
A couple of us have 2010 but mainly for the re-topo tools, which now that there's 3DCoat and a re-topo script that works in 2009 basically means we only use 2009 as our primary 3D application.
Thanks!

2011 x64 missed the subscription so no 2012 for me, also didn't read much good about it. working with 2011 mainly because my clients use it, otherwise i would have stood with 2009
2010 at home and work, 2009 for a long time till I had to switch to 2010. I haven't yet touched 2011 and 2012 mainly because I don't have the time and major interest in the changes.
Since max 9 not really that much essential things have changed for me and I believe many others here. And once a software gets a reputation of not really bringing any real updates to the table you start loosing interest in catching up, especially if the updates consist of plugins of other studios that you checked out before (graphite modeling, mental ray, obj exporters,..).
they are still called dongles except these days they are USB ones instead of LTP port based, thing is they are expensive to produce hence why less companies use them these days.

Hehe yeah, the poll could start at like 3.1 or 4.2 maybe hehe. Still remember the 2.5 days on CG magazines cover CDs tho!
2009 @ home..
i think the 2k9 is the best 3dsmax too...
