Like almighty_gir stated here, it really depends on what you want. What kind of heightmap do you want? If you want nice displacement results, it will require baking from a high-poly mesh. If you just want something similar to a bump map, well then you can get that generated from a 2D diffuse map. Or just paint it yourself.

I'd like to take a crack at it, and maybe figure out why CB is giving you funky results. There might be something I can tweak to better handle your image! (Then again, there might not... but in any case I'd love to take a look and see where CB is going wrong!)
