Character customization help!

Hi everybody!

I'm new to this forum so I'm not sure this is the right place to post this question, but I hope you'll bear with me!

So my question regards a game project I'm currently working on, but for simplicities sake I'll relate the question to Diablo.

Anybody who has played Diablo knows that there is only a few characters to choose from, however, one can "customize" them by giving them a specific armor.

What I want to know is if anyone has any suggestion as to what the most efficient way of doing this is?
When playing Titan Quest I noticed that the character has a "base" where he is wearing a tunic, and all items seem to be stacked on top of that base. But what of the vertices that are now covered by another mesh, do we just forget about them or do I have to make a new arm in order to put a bracelet on it?

In case it matters at all I use Maya for the 3D work and Unity3D for coding..

So the question got a little messy after all, but I hope it is understandable and that someone here can help me.. :)
