Which is everyone using? I'm decent at drawing by now, but I much prefer to work with a mouse. I want to get into sculpting pretty badly, but I don't know if I should without buying a Wacom.
How much of a difference does using a tablet make in zbrush?

Simply put, there's about ten times the level of control just in the stroke shape alone, let alone the control from pressure, tilt, and so on. If you have ANY form of 2D art experience, that jumps up by another order of magnitude.
zbrush utilizes pen tilt? I thought that was mostly exclusive to Photoshop and Corel Painter.
But with mouse. It is doable. If you use Mouse for Zbrush be sure to enable lazy mouse.
Zbrush does not utilizes pen tilt but it does utilizes pen pressure I believe.

Ok first off, if you are planing on taking digital art seriously you will have to get your self a tablet at some point. ZB is designed to work with wacom tablets and is the only way to go there is honestly no point otherwise. Anyone who says different has never truly used a tablet. It gives you pressure sensitivity, stoke angle, and the fine tuned control of a pen.
Get one right away it will be the best thing you ever get your self as an artist! They are so much fun!
