My vote goes for Keyboard Shortcut Override being turned on, its the devils button...
It forces the shortcuts for sub menus to override the 3dsmax main shortcuts. So when you're working on vert mode in edit poly with KSO on, Ctrl-S will toggle soft select on and not save. It's a little button in the top menu that looks like a keyboard key with a T on it. Even if its off, toggle it on then off sometimes it sticks...
I'm not sure if its the same thing, but I've had an issue with 2009 after I installed the creative extensions that whatever I do to an imported/merged biped object, it isn't registered in the history. I haven't noticed if its spread to other things...
there's also an undo bug in 2008 that seems to discard the undo list randomly when doing animation related stuff. someone on the autodesk support site mentioned there would be a fix for this in a couple of weeks.

The history wipe has been around for a while hasn't it? I think I remember hitting it in max8-9. I think it happens when it autosaves? Glad to hear there's a fix coming!
