If I subtract or smooth I get this weird pixel crap instead of my true brush stroke. Any ideas? This is unworkable.

Try turning on "Quick" in the Transform panel.

Toggling the Quick option on and off does fix this. Better than rebooting which also seemed to fix it

Edit- I'm pressing F for Frame as a force of habit. This is the hotkey for Quick.
PS. How do you frame your current work area?

I think it's Del by default, however if you hold CTRL and click on the "Frame" icon you can assign any hotkey to it (provided you're on 3.1 since you can't do this in 3.0).

Alt + Left Mouse on the canvas, and the camera will focus on the visible meshes.
If you mean you want the camera to rotate around the area you're working on, you activate the Local button.

Ah the Local button. That's what I needed.
While we're at it here's another conundrum. Why is the Smooth tool double the size of the Standard brush and is there a way to disable that?

Yep, look in the Brush (or Stroke panel, I can never remember what it's called) and look for the "Alt Brush Size" slider - set this to 1.
