I wish importing and exporting would go perfectly with everything all the time, and sadly, that's not often the case. I thought I heard something was changed about the Blender file dialog. Was that in 2.46, and --dare I ask-- is it less terrible now?

There were some issues associated with doing a direct import and export of meshes (using Blender as a simple converter), usually OBJ files, where nothing was done to the objects whilst it was in Blender; not sure what goes on behind the scenes but it *would* do funny things like that (always has in fact, much like any other 3D app AFAIK).
There is talk of bigger changes for 2.50, not looked at 2.46 myself yet but I wouldn't be expecting too much of a change just yet on that one.
2.5 should have the interface upgrades your looking for. In addition to a new file browser and redoing all the buttons and menu layouts, they are also planning to add better Python/UI integration so perhaps there will soon be drop-in scripts to customize the interface any way you want.
