It's me again. One of my trouble spots is the transition to the shoulder, low or high resolution. I'd like a few examples if I may, to study some more proper layout. Here are some shots of the shoulder area right now:
Cap that arm off, and make it into the shoulder mass, then extrude downwards for the arm.
Have a look at your own arm, AT REST. People don't walk around with their arms out like that (T-pose). Modelling in the T-pose instead of an A-Pose can make it harder to get a model to hold it's mass when rigged.
