Source engine phong shader?

Anyone know how to write a .vmt file to use this? My googling has only yielded the "$phong" bit. I'd like to know how to give it paths to the spec map and occlusion map. A sample .vmt would be very helpful.
Also, does it only work on DX9 graphics cards? Does it need a shader fallback for lower-end cards? Thanks in advance for any help.
