I personally prefer Destruction first and then Death. That way, they will see how everything they love burns in hell and then will die in a more piggy way :P

I very much like the head, and the torso of your original sketch. Try taking out all the elements that look as if they were scribbled while extremely bored in algrebra class (axe in head, crossbones, buzzsaw, shoulder spikes, and the tattoo), and work from there. Those trivial details add nothing to the character. Focus on his form.
cape yes, I reckon you should see how far you can get away with pushing it.. think spawn, big fuck-off cape, and maybe some big unusually shaped spikes on his shoulders and down the outside of his legs. And a blinding green glow coming from inside his face. just some ideas
You have a good start. But, as mentioned, you better get moving. You may want to think about the designs for legs and such to give you the guidelines you need to work efficiently. Put something down, and work with it. Go!
