To contain male nudity: Witchaven, 1995 (the goblins having it were completely scrapped in the sequel for this reason, silly capstone)
To feature gun reloading: Fortress of Dr. Radiaki, 1994 (not only did it have reloading, but unloading as well haha!)
To feature drivable vehicles: Terminator Future Shock, 1995
To feature scientists: Blake Stone, 1993 (you could steal their coins too)
To feature health machines: Blake Stone, 1993 (to use the coins with!)
To feature destructive walls: Operation Bodycount, 1994.

I'd say Duke Nukem 3D, because Custer's revenge was a side scroller, not an FPS.
I remember Blake Stone, I think that was a game using the Wolfenstein source. Becuase it was basicaly Wolfenstein, but in SPACE!
I remember there used to be an FPS that also had Ninjas (as well as a sumo wrestlers, everthing had an oriental theme, and you could even use chop sticks as a weapon.
