Scale on wood seems a bit off. It's also a bit flat.

Do you think the scale needs to be bigger or smaller?

Do you have any tips on making it less flat?

Coming along nicely. Some parts look great, others seem a bit lazy.
E.g. the dirt on the wood is too uniform and the wood grain and dirt is also continuing through elements that would normally be built from separat pieces like the base or the stairs and the grain has unexpected directions. Inner and outer edges could use some wear or grime and partly beveling.
The blood looks a bit like paint to me. No idea how often they used to wash those things, but unless this is the very first kill of the day, there should be at least some older blood around and in less uniform quality and perhaps some soaked into the wood (as it doesn't look to be treated in your case, while you'd probably want to oil it or apply some kind of other finishing to avoid just that).

Some practical considerations (not terribly important, especially since you asked about texturing, but might help to make it look more real):
Would the wheel/windlass at the side have to turn when the blade falls? Seems like that could cause unnecessary friction and also be dangerous to the operator. Might make sense if there's a way to release the blade from the rope, but it seems to be tied to the blade.
The blade and the board it's attached to don't seem all that heavy and the rope having some slack (or being very stiff, also quite thick) adds to that impression. At the same time, the blade is probably not as heavy as to really need a windlass, but I understand it adds visual interest.
Showing some kind of pulley system might add some more visual interest, too, instead of simply having the rope disappear between the boards.
A  form of handle to open the board for the neck might be handy. Also, the hole seems much too big compared to the bench.
The stairs aren't up to code. Especially the last step seems much too short, again compared to the bench and they are basically blocked by the bench as well. Imagine having to carry an uncooperative poor sod up there and place him on the bench. It's an OSHA nightmare!
I'd say the proportions seem a bit off in general. First impression is solid, though.
