ZBrush 2021 startup not loading custom menu

I'm trying to install the Michael Pavlovich Zbrush startup. I've followed the simple readme, backed up the default Startup and unzipped the content into the Zbrush folders. All simple enough.
But for some reason the the Alt+A Custom Menu is not in the top tool bar and the Alt+A menu is not working. What I do find odd is the hotkeys for all of the brushes are there and working so ZBrush is detecting the startup it's just not pulling in the Menu.
Anyone have any ideas?
