Rugs/Carpets

How are rugs typically done? I'm going to be creating a Victorian rug and i'm wondering if Substance Designer is the way to go to create a complex pattern that will remain mainly flat, other than creases and folds that can be done using the geometry that the material will be on.

Back in the day, slapping a texture from the internet would be the way to go  I know there is better looking ways of going about it now.
