I'm currently trying to make an easy highpoly model from a lowpoly just by adding some bevels on hard edges for a game asset. As I tried it I encountered a whole new problem I can't find any information on in the forum.
Pictures are easier than trying to describe it. The Bevel Tool offsets the faces until a fractional amount of around 0.424. History is cleared, Transformations freezed and resettet, Bevel Tool resettet, and I also tried resetting the overall maya preferences.

Try using non-fractional bevel to get around it. And make sure your mesh is cleaned. If that doesn't work, try reporting the issue to Autodesk with help->speak back->report a problem...
I found the solution by doing other things first:
My model was pretty small and it seems like fractional beveling doesn't like that. So my recommendation for others who want to bevel small models: Scale up - Do the beveling - Scale down (or do it in blender which can handle it just fine)
I don't get why fractional beveling doesn't work with small bevels because it should just work with percentages but maybe Autodesk isn't getting this either
