How to copy a texture from hi-poly to lo-poly mesh?

In this simple example I have a square plane which I've subdivided into 10 Columns to fake a fence made of 10 vertical planks. Using a simple front-facing UV map, I can then separate the 10 UV islands to create discontinuities, which helps the illusion of separate planks.



Is there any way to create a new single texture which is the end result of what you see on the left?
I could then apply this to a single-poly plane to get the same effect.

I thought about chopping and rearranging a texture in Photoshop, but that would be very difficult as the normal and other maps would have to have the exact same cuts, and with more organic shapes would be almost impossible to line up with the edges of the hi-poly version.

It seems logical that if I can see the result (in any software package), there could be a way to save it?
