Zbrush Zmodeler Question!!! Edges separating and creating holes

Newbie question here.. When I Qmesh something like this some edges separate like this... What is the problem and how can I fix it? How do I weld the 2 edges together without creating another poly in between? Thanks!
Also, I've used close holes and fix mesh to solve the problem but I'd like to know how I can prevent this in the first place...
