Working with instances - from zbrush to game engine - workflow

Hi everybody, searched everywhere but couldn't find anything. 
What's your workflow with instances? 
I mean to save geo and texture , to instance things like buttons on a character or repetitive features on a facade .
Can you work with instances in zbrush or you don't arrange them together untill in engine space?
Or maybe im just really confused about the whole instance thing  :#
