Zbrush dynamesh spikes problem

Hello, im still learning zbrush and iv noticed that often this happens in my meshes. they get those spikes or sometimes holes that pierce the mesh. im working on dynamesh and dragging with ctrl to recalculate doesnt eliminate thise. neither does relaxing, smoith brush, fix mesh or close holes option. why do those spikes appear?

    What is your polyflow on the object ? Are you starting from scratch inside zbrush or are you importing a base mesh? This usually happens to me when I have Tri polygons because the dynamesh will clamp down and start to move up as the polygons get more and more dense in the tri. If you press Shift+F what does the geo look like?
