xnormal cages and vertex extrusions

It seems that sometimes when I use xNormal to create a cage, some of the vertices don't extrude along any axis unless selected and manually extruded. What causes this?

Is your mesh triangulated? Sometimes if you forget to triangulate it can push the verts out weird where you have a quad or polygon.
