I have an issue where the object disappears on screen using 3ds MAX 2015 (can't use 2012 version). I use the DirectX Shader and 3DS Max is on directX9. Any help is appreciated.
Try switching to Nitrous display instead (Preferences, change display driver). You shouldn't need to be in DirectX rendering mode to support using DX Shaders.
If this still doesn't work, check in the material editor to make sure there aren't any errors reported in the shader.
I followed the instruction carefully, and there doesn't seem to be in any kind of error in the material editor. This is one of the peculiar case where I can't find a solution in the forum
That is very odd. What hardware are you running this on? I have seen similar problems with people using a laptop that doesn't have a dedicated graphics card.
I updated my NVIDIA but the problem still there. On the side note I think 3dsMax is using NVIDIA as graphic card not intel (proof provides as an image below)

Here is the link to the customized material editor:

Go to NVIDIA Control Panel.
Manage 3D Settings.
Program Settings.
Add... 3ds Max exe.
Turn on settings, and see if this helps.
I can't recall which one, but one of the settings will force 3ds Max to use the Nvidia GPU instead of the weaker Intel GPU. Should solve your problem.
You could also try updating your Nvidia driver.
