When you sculpt on something thin, ZBrush will sculpt on the other side the opposite of what it sculpts on the one side. What you need to do if you don't want this to be the case is go to Brush->Auto Masking and click BackFaceMask, and you'll no longer have this problem. However, if you have a lot (>2mio) of points you'll notice that you get some slowdown when you rotate the model. This is because Zbrush has to recompute which faces are on the back-side of the model every time you rotate it around, and that can take a bit of time on a really dense model.
