Few questions about creating an environment in UE4

I watched the Unreal Engine 4 intro posted on their youtube channel, and noticed that their intro level was more of a use our floors to create a level, and throw a tileable texture on it. Cool for creating generic levels, but I have a few questions about more custom content.

questions:

1.) When starting a level, do you block it out in UE4 basic floors / walls / doors?

2.) When importing custom content, is there anything usually other than a .fbx I need to bring in?

3.) Do I need lightmaps? I haven't learned them yet but lightmaps are a bit confusing for me and if I need to learn it, I'll learn it.

Basically, I'm just curious if anyone kind of has a quick list of how they do things when creating an environment.

kinda like:
1.) block out in UE4
2.) import to maya
3.) create assets on top of blockout

etc.
