Wow, glad to hear that TQ is still being played. A bunch of us polycounters, myself included, worked on it. If you like TQ you might like Grim Dawn, which is being developed by TQ's lead designer, a dude named Arthur, using the TQ engine and is available for pre-order/beta access now.
There's that Van Helsing game that came out recently too that looked a little TQ-ish..anyone know if it was any good?
Path of Exile is your best bet by far. It's got everything you want in an action/hack and slash RPG. I've also been closely following Grimm Dawn for a while as well and its also looking very promising so be sure to check out the alpha or keep an eye on it going forward. Those are my top two hands down.
I went on a hunt for some Diablo clones a few months ago, you can find a few others here, I'm actually getting a kick out of Drakensang Online, its very Diablo/Titan Quest and you can play the entire thing in your browser, you can't beat that. I also loved Realm of the Mad God for the same reason, browser playability which was important for me (probably not for you).
I actually also got heaps of gameplay hours out Solomons Keep (iOS), its got none of the depth of Titan Quest but it kept me busy on the bus rides to work and has a very early Diablo feel to it.
Other games worth mentioning:
Torchlight (single player only)
Torchlight 2 (just as good as the original but with multiplayer!)
Dungeon Siege 2 (best of the series in my opinion, just ahead of the first game, easy to get into and also has a great Titan Quest vibe to it).
Fate (bit to simple for my tastes but still a good hack and slash game for a casual option).
Sacred series (I preferred 2, the game was just average though)
Well, there are a lot of games like Titan Quest. Besides those mentioned in the thread, I'd recommend Divinity and Divinity 2 (which is in an early access now)

I totally agree with you also Path of Exile is having a race event with 5 Chinese vs 5 Western players on March 10th. This event will feature 5 Chinese and 5 Western streamers who are known for racing. They are racing to Kitava solo, so this is not a 5v5 team race. Winner is getting $3000, 2nd gets $1500 and 3rd $750:
Yeah... me, Josh, Bobo, KillingPeople, Parnell/ModelingMan, Tarwater, Sectuars...for such a small studio..there were a lot of polycounters there. Also, Grim Dawn is basically as close to an endorsed followup that you can get: Same lead designer as Titan Quest, same engine, some of the same developers..
Also, I heard TQ was rereleased by Nordic with an entire new chapter, build from existing stuff in the game..but I never tried it.

But if you're the type of person that cares a lot about customizing character appearance, like I'm sure most artists would, it will anger you. 99% of the character's appearance choices in the game are relegated to the cash shop, which is absurdly expensive. A single outfit, for a single character, will run you like $50.

That's...disappointing. I am about to finish a playthrough of Diablo 3 and had PoE all downloaded and prepped to go to continue feeding my addiction. My excitement level just dropped immensely
